Student enrollment must be completed online. Please submit the items listed in the Enrollment Checklist to start the enrollment process.
Registering your child ensures that you will receive important information and updates from AGMS. If you have any questions about the enrollment process, please contact our Registrar, Mrs. Geraldine Guzman at 980-343-5810.
CMS Mandatory Medical Forms
Students that require medical attention during school hours are required by CMS to have proper documentation. This documentation allows school and medical staff to treat your child. Please note that these medical forms must be filled out by BOTH parent and physician.
Scans or clear photos of completed Medical Forms must be emailed directly to BOTH the Registrar mailto: geraldine1.guzman@cms.k12.nc.us and the School Nurse melissa1.mcgreal@cms.k12.nc.us
